What You Must Have:
- Bachelor’s Degree in nutrition or related subjects, preferred
- Current State Driver’s License, reliable transportation, and provide ongoing valid and current auto liability insurance
- Two years of experience as a Dietician in a clinical care setting, hospice or home health preferred.
- Ability to establish and maintain effective working relationships with all segments of the staff, the lay and professional public, the Board of Directors and Medical Director.
- Understands hospice philosophy, focus of care is comfort vs. rehab and to maintain care in their residence as long as possible
What You Will Do
- Provide direct dietary assessment of patient
- Ability to establish plan of care to meet the dietary needs of the patient
- Provide education to patient and caregiver regarding diet management
- Appropriately report changes and document pertinent information and care rendered to patient to ensure continuity of care.
- Provide counseling and education to IDG, nursing staff and physician, as appropriate
