Clinical Liaison Registered Nurse


What You Must Have:
  • Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
  • Current State license as a Registered Nurse
  • Valid driver’s license, reliable transportation, and provide valid and current auto liability insurance
  • Two years of experience as an RN, preferred in hospice or healthcare/clinical setting

What You Will Do:
  • Develop a Hospice presence in the healthcare community with hospitals and health care providers
  • Performs comprehensive nursing assessment and evaluation for appropriateness of hospice admission
  • Ability to coordinate care amongst the Hospice team
  • Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities
  • Develop, educate and consult with medical professionals concerning hospice programs, pain management and Medicare/Medicaid/commercial reimbursements
  • Inform appropriate patients and their families about Hospice goals, philosophy and services we provide to them
  • Actively establish and maintain market acceptance and allegiance
  • Establish public relations programs for interpretation of the agency's services and to foster good working relations with physicians and community agencies. Responsible for the implementation of specific projects and initiatives as directed
  • Responsible for new business development and assisting Community Education Representatives with new account development when needed
  • Work closely with agency and sales leadership to meet growth and development targets
  • Ensure hospice plan of care is adhered to during admissions
  • Assist and provide explanation of hospice services and documentation
  • Flexibility in work schedule as referral source, patients and families may require after hours and weekend consultations
  • May be asked to perform patient care visits, On Call or assist in RNCM visits as needed
  •  Timely submission of all documentation
  • Conduct all business activities in a professional and ethical manner
  • Performance of other duties as required
  • Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
