Patient Care Manager - RN

Salary: $135,000 - $145,000

You will be working with Lisa Stubenrauch RN BSN, Executive Director along with another experienced Patient Care Manager.

What You Must Have:

Current State Registered Nurse License
Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance
5 or more years' experience as an RN with 2 or more years' experience in hospice.
Prefer RN with prior supervisory experience managing other nurses.

Must be highly motivated, self-directed, flexible, and able to work well and participate in a multidisciplinary group setting
Specialized skills preferred in palliation and end-of-life
Proof of current CPR and maintain a current status
Must meet mandatory CE requirements of licensing board per state and maintain a current status
EMR experience a plus
