Position Title: Reentry Case Manager
Pay Rate: $19-$22/hr. depending on experience


Position Overview:
The Reentry Case Manager is responsible for providing direct case management services to clients at the Hope Works Program through Hope Initiatives. This role aims to enhance public safety and decrease recidivism by helping clients become stable in the community and desist from criminal behaviors.

Key Responsibilities:
  • Manage a caseload of approximately 30 clients, assisting with issues related to benefits, employment, housing, childcare, healthcare, obtaining identification, and other supports to increase client success.
  • Work with clients to create support groups and pro-social recreational activities.
  • Maintain positive professional relationships with staff at Monroe County Probation, the Monroe County Jail, the NYS Department of Corrections and Community Supervision (Parole), and other relevant governmental offices.
  • Maintain accurate and timely client records.
  • Participate in community meetings as needed and build robust working relationships with community agencies
  • Perform other tasks as assigned.
Requirements:
  • Demonstrated commitment to working as part of a multi-cultural staff to provide appropriate services to a diverse client population.
  • A Bachelor’s or Associate’s degree in social work, criminal justice, psychology, counseling, or a closely related field is preferred. Relevant experience working with clients involved in the criminal justice system, mental illness, and/or addiction may substitute for a degree.
  • Personal experience with incarceration or having a family member who has been incarcerated is a plus.
  • Reliable transportation and a clean driving license.
  • Ability to pass a clearance to enter the Monroe County jail is preferred.

Desired Skills/Abilities:
  • Ability to successfully engage clients, including those with serious, persistent mental health issues and criminal justice histories.
  • Understanding of the effects of trauma.
  • Proficiency in Microsoft Office and ability to utilize a Microsoft Access-based case management system.
  • Familiarity with community resources that meet a wide variety of client needs.
  • Comfort in working independently.
  • Bilingual abilities are a plus.
  • Certification in Offender Workforce Development (OWDS), Moral Reconation Therapy (MRT), Thinking 4 Change (T4C), and/or CASAC is desirable.
