Position: Program Coordinator – Care Parent Network (CPN)
Salary: $72,540 annually
Location: Concord, CA – Providing services Contra Costa County
Status: Full-time, Exempt
VistAbility is looking for a Program Coordinator for our Care Parent Network. This role supports families of children with intellectual and developmental disabilities (IDD) in the community. This role leads the daily work of the Care Parent Network (CPN), provides guidance to families, supports staff, and helps strengthen partnerships in the community.
Responsibilities:
Family Support & Program Coordination
• Oversee daily CPN operations, including parent support, information & referral, trainings, and resource navigation.
• Conduct intakes, needs assessments, and service planning for families.
• Build ongoing relationships with families, Regional Center case managers, and community partners.
• Represent CPN at community events, outreach activities, and collaborative meetings.
Program Quality & Compliance
• Monitor service delivery, family satisfaction, and documentation quality.
• Recommend and implement program improvements.
• Identify barriers to access and contribute to program development.
Staff Leadership
• Provide orientation, training, guidance, and supervision to assigned staff.
• Conduct performance evaluations and recommend personnel actions as needed.
• Support staff development and ensure training requirements are met.
Administrative & Operational Duties
• Coordinate workshops, special events, and community trainings.
• Maintain accurate records, documentation, and program reports.
• Serve as program lead when the Director of CAS is unavailable.
• Collaboration with Development Team
• Provide data, stories, and documentation needed for grants and funder reporting.
• Ensure program activities align with funded goals and deliverables.
Requirements:
• At least 3 years of experience supporting families of individuals with IDD.
• At least 1 year of supervising staff or volunteers.
• Lived experience: Must be the primary caregiver of a child with an IDD (current or past).
• Strong communication, organization, and teamwork skills.
• Ability to use email, Microsoft Office, and basic data systems.
• Valid CA driver’s license, good driving record, and an insured vehicle.
