Position Summary
The purpose of the IRTS Coordinator position is to provide education and training to clients in the IRTS facility and to provide case management services. The work for this position is generally assigned by the supervisor and tends to be general instructions about the assignments to be completed. The person in this position generally makes operational decisions that affect the day-to-day functioning of the program. The IRTS Coordinator interacts with a number of external entities including:  previous treatment centers, county financial assistance programs, and local law enforcement.

Essential Job Functions
Facilitate groups and support services with clients (30%)
  • Psychosocial education groups
    • Assist with medication education as needed
  • Basic living skills groups
    • Assess each client’s basic living skills
    • Monitor meal preparation and may assist as needed
    • Set individualized cooking goals with each client using a variety of techniques including hands-on assistance in helping each client achieve their goals
    • Monitor nutrition, oversee menu-planning, food storage, and grocery shopping; ensure customary standards are met
    • Monitor house, kitchen, and bedroom cleanliness; provide guidance, as needed
  • General support services
    • Meet with clients regarding progress in meeting individualized program goals
    • Connect with clients in a daily, less formal basis to check in
    • Assist clients with clinical problem-solving, including crisis prevention and resolution
    • Assist clients with financial management, mobility/transportation issues, health and medical care needs, housekeeping, nutrition, grooming, hygiene, social and leisure skills
    • Ensure follow-through on all planned interventions and reports problems in this area to appropriate personnel.
    • Encourage clients’ participation in a routine of productive daily activities

Case coordination for caseload of three to five clients (20%)
  • Facilitate training plan reviews and discharge meetings, as scheduled
  • Ensure that the client's orientation to the program is complete
  • Maintain awareness of client concerns; knowledgeable about client's performance in all areas
  • Develop individual treatment plans with clients; ensure client's active involvement in own program planning; assessment of progress; problem-solving
  • Serve as an advocate for assigned clients; liaise to other involved service providers
  • Facilitate the development and implementation of client's discharge plan



Monitors medication group (10%)

Documents client interventions and completes other required paperwork (15%)

Communicate and collaborate with multidisciplinary team regarding client functioning (10%)
  • Collaborate with other Training Center staff in problem-solving training related issues
  • Communicate with all appropriate Tasks staff regarding client’s functioning and with IRTS Nurse regarding clients’ reports of effectiveness of medication and medication side effects

Other duties as related to the position (5%)
  • Contact prior treatment centers to obtain client’s previous treatment history
  • Contact county departments regarding client’s benefits paperwork
  • Research resources for alternate placement prior to client’s discharge from program

Qualifications
Education and Experience
Required
  1. Bachelor’s degree in psychology, social work or related field
  2. At least one year experience working with adults who have been diagnosed with a serious and persistent mental illness
  3. Valid driver’s license

Preferred
  1. Fluent in a non-English language or competent in the culture of an ethnic group to which 20 percent or more of the agency’s recipients belong
