GENERAL JOB SUMMARY
  • This position is responsible for providing end-of-life medical social services for patients and their families. The Social Worker will assess the individual’s and family’s needs with the purpose to estimate their ability to cope with terminal illness and death and to help remove barriers to their access to care. The Social Worker will also assess the individual’s needs through administering an initial bereavement risk assessment.  Bereavement services may be offered prior to the death when the assessment identifies the need for the patient and or family. The Social Worker may meet with persons in their home or the facility at which they reside. The Social Worker will provide services within their scope of practice. This position does not carry supervisory responsibilities.
ESSENTIAL FUNCTIONS
  • Ensure that Hospice philosophy of care as defined by state and federal regulations is appropriately implemented.
  • Provide initial and comprehensive psychosocial assessments and develop care plans that meet the needs of the patient, family and their significant others focusing on the psychosocial, spiritual, and cultural factors that may impact their ability to cope with the dying process.
  • Provide the initial bereavement assessment upon the patient’s admission to determine the needs of the patient’s family and other individuals focusing on the social, spiritual, and cultural factors that may impact their ability to cope with the patient’s death.
  • Will take into consideration the family’s coping issues such as history of previous losses, family problems, financial concerns, communication issues, drug and alcohol abuse, health concerns, mental health issues, legal concerns, presence or absence of support system, feelings of despair, anger and guilt of abandonment.
  • Provide psychosocial counseling and support to the patient and family.
  • Provide accurate documentation of the initial and ongoing psychosocial assessments.
  • Provide documentation from the initial bereavement assessment and incorporate it into the bereavement plan of care and complete progress notes as required by hospice guidelines in a timely manner.
  • Complete and maintain electronic medical records and progress notes as required by hospice guidelines in a timely manner.
  • Assess the patient’s financial and insurance status and assist them with obtaining community resources as indicated i.e. personal business, financial, legal housing, advance directives, etc.
  • Assist the family with obtaining information about choosing a residential care facility without making any individual recommendations.
  • Help the family caregiver with finding resources for respite when indicated.
  • Provide the patient or family with verbal or written resources that may be helpful to their end-of-life experience as appropriate. 
  • Provide effective anticipatory grief and grief counseling to patients and their families taking into consideration the individual family’s beliefs, practices, religion, faith, customs, culture and ethnicity.
  • Provide short term-crisis intervention when necessary making appropriate referrals to community services such as local police and APS; following state and local laws and agency policies when indicated.
  • Assist with special events such as life celebrations, and memorials as assigned.
  • Collaborate and communicate effectively as part of the team to establish and maintain cooperative working relationships.
  • Participate as a member and attend IDT meetings as required to establish, review and revise patient plans of care.
  • Attend all in-service trainings as required.
  • Attend MSS department meetings as required.
  • Provide counseling in a respectful and dignified manner.
  • Practice safety in the workplace.
  • Maintain patient confidentiality at all times.
  • Advocate for the best interest of the patient and their families.
  • Keep the MSS Director fully informed.
  • Fluent in the English language.
  • Ability to position self to move about, ascend, descend or traverse, transport, operate, activate, use, prepare, inspect or detect, perceive, identify,  recognize, observe, inspect and assess are required. Ability to convey, communicate, speak clearly and exchange information, converse with, discern, and express oneself.
JOB REQUIREMENTS AND QUALIFICATIONS
Education:
  • Bachelors or Master’s degree in Social Work from and institution accredited by the Council on Social Work Education.
  • If a bachelor’s degree in social work the BSW must be minimally supervised by a MSW.
Training Requirements: (Credentials, licenses or certificates)
One of the following: BSW, MSW, LBSW, LMSW, or LCSW
Experience:
  • Minimally one year clinical experience in a health care or mental health setting.
Knowledge Requirements:
  • Hospice Philosophy of care; working knowledge of current hospice guidelines.
  • Human behavior and the procedures for assessment of psychosocial issues.
  • Group behavior and dynamics; societal trends and influences.
  • Stages of grief, death and dying.
  • Community resources including Medicaid, Medicare, financial aid, county, state and social service institutions, Veterans Administration, Arizona Long Term Care, and AHCCC’s benefits.
  • General knowledge of crisis intervention techniques and community disaster response.
  • Assume responsibility for own professional development.
  • Knowledge and support of agency’s safety and OSHA policies.
  • Understanding of HIPAA Privacy Standards and related agency policies.
  • Demonstrated understanding and compliance with agency policies, including but not limited to documentation, continuing education, work schedules and required HR documents.
  • Signs and symptoms of suicide ideation.
  • Signs and symptoms of substance, drug and alcohol abuse.
