Position:                Nurse Practitioner (NP)

Position Summary:
The Nurse Practitioner (NP) is responsible for leading the provision of comprehensive geriatric services as part a collaborating member of an inter-professional Geriatric Assessment Intervention Network (GAIN) Community Team. The NP provides direct patient care focusing on health promotion, maximizing patient safety and function to support frail seniors living at home

Principal Responsibilities and Duties:
1. Provide Clinical Care and Consultation Services (Assessment, Diagnosis, Care Plan Implementation, Evaluation)
  • Provide Care within the legislated requirement for a full NP Scope of Practice
  • Provide direct care focusing on health promotion and maximize patient safety and function to support frail seniors living at home
  • Deliver person-centered care to all patients in accordance with principles of the Behavioural Support Ontario (BSO) project

2. Plan, promote and coordinate evidenced- based care for older adults and frail seniors with complex needs
  • Plan, promote and coordinate evidenced- based care for older adults and frail seniors with complex needs 
  • Participate in regular meetings with the Director of Care Services and the inter-professional team to assist in program development and ongoing monitoring, policy development, participation in Quality Improvement and Safety and evaluation
  • Lead education and knowledge transfer to all clients and caregivers regarding care plan based on CGA (Comprehensive Geriatric Assessment)

3. Ensures patient safety and staff safety in all aspects of responsibilities
  • Complies with the SPLC “Client Safety”, “Risk Management”, “Inclusion” and “Health and Safety” policy statements and regulated requirements of affiliated professional association.

4. Establishes and promotes health and safety standards
  • Ensures compliance with patient safety responsibilities, internal policies, procedures and quality management

Education:
  • Baccalaureate Degree in Nursing, and successful completion of the Ontario Primary HealthCare Nurse Practitioner Certificate Program.
  • Membership in good standing with a regulatory body in Ontario (College of Nurses of Ontario, Nurse Practitioner Association of Ontario)
 
Experience and Skills:
  • Three to Five (3- 5) experience working with older adults and frail seniors
  • Two (2) years recent experience as a Nurse Practitioner in community/health care of older adults and seniors. 
  • Experience/ familiarity with RAI documentation including assessments, Resident Assessment Protocols (RAPS) and care planning functions
 
Others:
  • Required to work occasional evening and weekend hours
  • Required to work at satellites sites, home visits with clients and caregivers, working alone in the home
  • Must have access to a vehicle and a valid driver license
  • Vulnerable Sector screening required
  • Proof of full COVID-19 vaccination is required
