Position Summary:
The Pharmacy Clinical Specialist is responsible for providing advanced clinical pharmacy services to optimize medication therapy for patients. This role includes conducting both reactive and proactive medication reviews, managing pharmacy protocols, and ensuring safe and effective use of medications. The Pharmacy Clinical Specialist also supports medication reconciliation activities and formulary processes while educating pharmacy staff, nurses, and medical staff on best practices. Additional duties include precepting pharmacy students and residents, contributing to organizational quality improvement, and supporting the hospital’s commitment to safe, innovative, and personalized pharmaceutical care.

Key Responsibilities:
- Patient Care & Collaboration
- Conducts comprehensive medication reviews, ensuring safe and appropriate pharmacotherapy.
- Works with interdisciplinary teams to achieve patient-specific therapeutic goals.
- Acts as a preceptor for pharmacy students and residents, promoting clinical skill development.
- Medication Management
- Processes medication orders with attention to patient details (diagnosis, allergies, etc.).
- Provides drug information to support clinical decisions and contributes to medical staff committees.
- Quality & Safety
- Collaborates with healthcare providers to manage medication therapy, minimizing side effects and interactions.
- Participates in quality initiatives like adverse drug reaction reporting, inventory management, and clinical interventions.
- Education & Innovation
- Develops and presents educational programs for staff on relevant clinical topics.
- Engages in research and quality improvement projects, contributing to advancements in care and pharmacy practice.
Qualifications:
- Education:Graduate of an accredited program required for the certifications or licenses listed below.
- Experience:
- Completion of an ASHP-accredited PGY1 residency or equivalent experience (3 years of clinical pharmacy per residency year).
- Completion of PGY2 or fellowship training is preferred.
- At least three years of clinical practice experience.
- Certifications: Must meet qualifications to become an ASHP-Qualified Residency Preceptor within one year of employment.
