GENERAL JOB SUMMARY

 
  • This position will coordinate and supervise approved programs. The MSS Manager will be involved in researching and analyzing community needs, and facilitating approved programs. The Manager will work with the Director in helping design programs, as directed by the Director, to meet those community needs. Working with the MSS Director and staff, the MSS Manager will support the MSS Director’s implementation of programs, supervise, evaluate program goals and analyze data to determine effectiveness. This includes reporting findings to the MSS Director timely and as needed. The MSS Manager provides tactical management of all of the MSS departments. Will organize and coordinate the activities of the MSS departments by utilizing available resources to meet patient and organizational need.  Manage, supervise and evaluate the work activities of all of the MSS department teams. The MSS Manager is responsible for providing end-of-life medical social services for patients and their families with 24-hour, day-to-day operations accountability and availability.  The MSS Manager will assume daily responsibility for appropriate and adequate staffing, planning, organizing and managing Hospice of Havasu’s patient psycho-social services, bereavement and chaplain services. This position carries day-to-day supervisory responsibilities.
ESSENTIAL FUNCTIONS
  • Ensure that Hospice philosophy of care as defined by state and federal regulations is appropriately implemented.
  • Provide care consistent with the National Hospice and Palliative Care Organization standards of practice.
  • Establish weekly and/or daily assignments for social services team members, according to workload, to provide advocacy, referral, problem-solving and intervention functions related to personal, family, business and financial issues related to the patient care plan. This includes participation in IDT, and other organizational meetings affecting patient and caregiver’s care.
  • Plan, organize, coordinate, develop, implement, and manage the day-to-day needs of Hospice of Havasu’s patient social work, psycho-social, bereavement and chaplain services, under the direction of the MSS Director.
  • Verify pay sheets /payroll for MSS staff for both the Lake Havasu Main Office and the Fort Mohave office.
  • Influence (motivate, manage, direct) and supervise day to day the social worker, bereavement and spiritual program staff at both office locations.
  • Provide training for staff on AHCCCS, ALTS, HPOA, Living Will, VA and other community resources.
  • Prepare and present annual job performance evaluations for MSS staff in a timely manner.
  • Assist with updating job description for MSS staff, under the direction of the MSS Director.
  • Administer and evaluate goals for the MSS department.
  • Identify problems, recommend and implement solutions and evaluate outcomes with the MSS Director.
  • Investigate patient complaints, file reports, and inform the appropriate individuals or authorities as required by law, keeping the MSS Director fully informed in a timely manner.
  • Investigate incidents or unusual happenings, file reports and inform the appropriate individuals as directed by the MSS Director notify the authorities as required by law.
  • Participates in the Quality Assessment Performance Improvement Program and other organizational committees i.e. ethics, safety, etc. as requested by MSS Director.
  • Participate in the initial and ongoing training and orientation of new MSS employees, in coordination with the MSS Director.
  • Demonstrate experience in group dynamics, grief and loss dynamics, bereavement experience or credentialing, and a general overview of chaplain spiritual dynamics as it pertains to Hospice care.
  • May attend with MSS Director regularly scheduled staff meetings at the Lake Havasu City, Ft. Mohave and Polidori House offices identifying concerns; ensuring discussions are meaningful and valuable.
  • The MSS Manager may require doing direct social work and bereavement patient care as needed and necessary, under the direction of the MSS Director. Those duties include but are not limited to the following:
  • Provide initial and comprehensive psychosocial assessments and develop care plans that meet the needs of the patient, family and their significant others focusing on the psychosocial, spiritual, and cultural factors that may impact their ability to cope with the dying process.
  • Provide initial bereavement assessments upon the patient’s admission to determine the needs of the patient’s family and other individuals focusing on the social, spiritual, and cultural factors that may impact their ability to cope with the patient’s death.
  • Will take into consideration the family’s coping issues such as history of previous losses, family problems, financial concerns, communication issues, drug and alcohol abuse, health concerns, mental health issues, legal concerns, presence or absence of support systems, feelings of despair, anger and guilt of abandonment.
  • Provide psychosocial counseling and support to the patient and family.
  • Provide accurate documentation of the initial and ongoing psychosocial assessments.
  • Provide documentation from the initial bereavement assessment and incorporate it into the bereavement plan of care and complete progress notes as required by hospice guidelines in a timely manner.
  • Complete and maintain electronic medical records and progress notes as required by hospice guidelines in a timely manner. This includes assistance with employee data entry to ensure completion.
  • Assess the patient’s financial and insurance status and assist them with obtaining community resources as indicated i.e. personal business, financial, legal housing, advance directives, etc.
  • Assist the family with obtaining information about choosing a residential care facility without making any individual recommendations.
  • Help the family caregiver with finding resources for respite when indicated.
  • Provide the patient or family with verbal or written resources that may be helpful to their end-of-life experience as appropriate. 
  • Provide effective anticipatory grief and grief counseling to patients and their families taking into consideration the individual family’s beliefs, practices, religion, faith, customs, culture and ethnicity.
  • Provide short term-crisis intervention when necessary making appropriate referrals to community services such as local police and APS; following state and local laws and organizational policies when indicated.
  • Assist with special events such as life celebrations, and memorials as assigned.
  • Collaborate and communicate effectively as part of the team by face-to face, telephone conferences, and discussion to establish and maintain cooperative working relationships.
  • Participate as a member and attend IDT meetings as required to establish, review and revise patient plans of care.
  • Attend all in-service trainings as required.
  • Provide counseling in a respectful and dignified manner.
  • Practice safety in the workplace.
  • Maintain patient confidentiality at all times.
  • Advocate for the best interest of the patient and their families.
  • Keep the MSS Director fully informed.
  • Fluent in the English language.

JOB REQUIREMENTS AND QUALIFICATIONS

Education:
  • Master’s degree in Social Work from and institution accredited by the Council on Social Work Education, with at least three (3) to five (5) years of clinical experience in medical and/or mental health setting.  
Training Requirements: (Credentials, licenses or certificates)
  • Must minimally have a MSW /LMSW, LCSW preferred.
Experience:
  • Three (3) to five (5) years’ clinical supervisory experience in a health care setting.
  • Must minimally have two (2) to five (5) years’ in Hospice care, dementia medical field, mental health, and drug/alcohol family dynamics.
  • Must possess working knowledge in grief and bereavement dynamics.
Knowledge Requirements:
  • Hospice Philosophy of care; working knowledge of current hospice guidelines.
  • Business and management principles involved in resource allocation, leadership techniques, strategic planning and coordination of people.
  • Principles and methods for curriculum and training design, teaching and instruction for individuals and groups.
