Palliative Care Social Worker - PRN



Qualification and Education Requirements 
  • Bachelors or Master’s Degree in Social Work from a State accredited program
  • License in Social Work in the state you will be working
  • Valid driver’s license and car insurance
  • One year of experience as a Social Worker in a clinical care setting required, hospice experience preferred

Essential Duties
  • Assess patient to identify psychosocial, grief, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals
  • Provide individual patient/family counseling, crisis intervention, information and assistance with preparation of advance directives, funeral planning issues and transfer of responsibilities related to legal and fiscal issues
  • Facilitate community resources based on patient/family needs
  • Participate in all mandatory staff in-services and meetings
  • Maintain compliance with internal annual education requirements 
  • Timely submission of all documentation
  • Conduct all business activities in a professional and ethical manner
  • Performance of other duties as required
  • Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
