Community Education Representative - Post Acute Specialist

What You Must Have:
  • Bachelor’s Degree in Marketing, Business Administration, or a health related field or equivalent; preferred
  • At least 3 years of progressive sales experience in hospice sales or other health care sales strongly preferred with particular knowledge / experience in the post-acute market space (SNF, ALF, PCH, Memory Care)
  • Knowledge of business management and a strong knowledge of the Medicare Hospice Benefit and skilled nursing payer benefits
  • Must have an outstanding interpersonal, communication and presentation skills
  • Ability to establish and maintain effective, professional working relationships with all segments of the staff, the lay and professional public, the Board of Directors and Medical Director
  • Current State Driver’s License, reliable transportation and provide ongoing valid / current auto liability insurance

What You Will Do:
  • Special focus on post-acute patient service locations (skilled / long-term care nursing facilities, assisted living facilities, personal care homes and associated care providers i.e. physician’s and extenders).
  • Call on skilled nursing facilities, assisted living facilities, personal care homes, memory care facilities and associated physicians and extenders within an assigned territory to open new accounts, develop existing relationships and promote hospice referrals and community resources
  • Develop and implement territory business plans and manages sales time and resources according to plan, adjusting as necessary in order to satisfactorily complete the plan and meet established goals
  • Implement and maintain best practices in areas such as hospice sales, including territory and account management, prioritization and call volume, sales closing, and relationship building
  • Utilize customer relationship management tool, RSL and other tools provided to effectively manage accounts and performance
  • Continuous pursuit of knowledge about hospice services and Heart to Heart to Heart Hospice to become a trusted advisor to educate referral sources, patients and internal staff about hospice and hospice services
  • Establish and maintain ongoing public relations efforts to increase brand awareness and community understanding of Heart to Heart Hospice and its services
  • Meet regularly and work closely with Heart to Heart Hospice local/regional management to discuss sales activities, new opportunities, customer service opportunities
  • Flexibility in work schedule as referral source, patients and families may require after hours and weekend consultations
  • Perform on-call responsibilities and provide on-call service to patients/families as assigned
  • Adhere to company HIPAA privacy protocols and compliance programs
  • Utilizes excellent customer care skills with internal and external customers, including using proper phone etiquette
