The applicant hired for this position will coordinate and monitor services for consumers in the HCS program according to the Consumer's Individual Person Directed Plan
Duties include:

  • Develop IPC and Implementation Plans based on outcomes identified in an individual's PDP;
  • Coordinate and monitor services received from internal and external service providers;
  • Complete IPC's, Implementation Plans and other required documentation according to HCS standards, HHSC and HOTRMHMR's guidelines;
  • Monitor consumer's progress on objectives, and modify as necessary-document progress quarterly;
  • Monitor consumer's financial accounts, approve personal spending, determine PNA amount and coordinate spend-down shopping;
  • maintains cooperative working relationships with staff and consumers;
  • maintain specific documentation hours per month.

Knowledge, Skills, and Essential Functions

  • Excellent written communication  and advanced computer skills using Microsoft Office and other programs;
  • Ability to provide accurate documentation;
  • Ability to provide leadership without a lot of direct supervision;
  • Ability to make decisions and exercise good sound judgment;
  • Ability to maintain a wholesome, caring attitude;
  • Excellent verbal and written communication skills with consumers, staff, and community providers;
  • Ability to maintain good daily attendance and have good time management;
  • Stooping, bending, standing for prolonged periods and occasional lifting up to 50 pounds;
  • Ability to be CPR/First Aid and PMAB certified;
  • Ability to maintain a level of general health that permits performance of job duties;
  • Valid driver's license, liability insurance, and reliable transportation required.

Education and Experience

  • High school diploma or GED Required
  • 1 year work experience with individuals with developmental disabilities required, however, college coursework pertaining to rehabilitation therapies may be substituted for experience on a basis of fifteen hours pertinent course work for six months of experience.

Hours: 8AM-5PM Monday thru Friday - With notice may need to vary hours

Salary: $10.00 hourly/ $20, 800 annually
