This is a contracted position through Heart of Texas MHMR.

The Host Home provider will provide supervision, support and training to an adult individual residing in the provider’s private home. The Host Home provider is responsible for the care and well-being of the individual by ensuring that their daily needs are met in a family home setting.  This is an excellent opportunity for a caring person to work with individuals with intellectual and developmental disabilities in all areas of daily living skills and community involvement.

EXAMPLES OF WORK PERFORMED:

 
  • Provide assistance with activities of daily living, including bathing, dressing, meal preparation, housekeeping
  • Provide individual training and supervision in performing activities that foster greater independence
  • Provide  opportunities to participate in activities both in the home and in the community
  • Maintain records of individuals’ participation in and responses to training, treatment and everyday activities
  • Provide administration and supervision of individuals’ medication.
  • Provide transportation to medical appointments.
  • Provide transportation to and from Day Habilitation daily.  
  • Complete accurate documentation, including weekly progress logs, emergency drills, medication records, etc

GENERAL QUALIFICATION GUIDELINES:

Experience and Education:

A high school diploma or equivalency, or may complete a written competency-based assessment and provide three written references. Experience with people with Intellectual Disabilities is preferred but not required.

Knowledge, Skills, and Abilities:

The person in this position must have the desire and ability to provide ongoing supervision of individuals, to provide assistance with daily living skills, to provide training to achieve goals, to complete accurate documentation, and to communicate effectively both orally and in writing.

Registration, Certification, or Licensure:

The person in this position must have reliable transportation, a valid driver’s license and current automobile insurance.
 
Physical and Emotional:

Must have and maintain a level of general health that permits the performance of required duties. Must be free of communicable disease. The person providing Host Home care must be able to successfully participate, perform, and complete the skills required for CPR, First Aid, and PMAB certification.

Salary: Day Rate Based on Level of Need (Day Rate X 365)
