Habilitation Specialist works with Service Providers to provide supervision and assistance to consumers in life skills training.
Duties include:
- transporting consumers to and assisting them with activities;
- functions as IDT member by providing input regarding client needs and progress in interdisciplinary meetings;
- completes assessment for individuals referred to or in community support services;
- recommends training priorities;
- writes methodologies and monthly training sheets, collects monthly training data and prepares quarterly reviews;
- maintains schedules for direct support professionals;
- completes annual reports to distribute to IDT members for review;
- maintains cooperative working relationships with staff and consumers;
- maintain specific documentation hours per month.
Knowledge, Skills, and Essential Functions
- Excellent written communication and basic computer keyboarding for documenting training using Microsoft Office and other programs;
- Ability to develop habilitation plans and provide accurate documentation;
- Ability to make basic decisions and exercise good sound judgment;
- Ability to maintain a wholesome, caring attitude;
- Excellent verbal communication with consumers, staff, and community providers;
- Ability to maintain good daily attendance and have good time management;
- Ability to transport consumers locally 75% of time and regional travel 25% of time;
- Ability to sit in a car for prolonged periods of time;
- Stooping, bending, standing for prolonged periods and lifting up to 30 pounds;
- Ability to be CPR and PMAB certified
- Valid driver's license, liability insurance, and reliable transportation required
Education and Experience
- High school diploma or GED Required
- 1 year work experience with individuals with developmental disabilities strongly preferred
- Valid Driver's License and proof of liability insurance Required
Hours: 8AM-5PM Monday thru Friday - With notice may need to vary hours
Salary: $10.00 hourly/ $20, 800 annually
