The person in this position will develop and coordinate a wide range of activities to interest and stimulate the physical and mental well-being of individuals with intellectual developmental disabilities in a Day Habilitation setting. The person in this position will train and motivate the Direct Care Professionals that work directly with the individuals who participate in the program each day. The position will require the ability to show flexibility in scheduling, independent judgement and the ability to problem-solve. Must work cooperatively and effectively with other center staff/professional; to exercise professionalism, initiative, and good judgement when working with individuals in the program; to prepare reports and recommendations for administrative use; and to communicate effectively with individuals and their families/guardians.
• Developing activities and activity schedules that address the wants and needs of the individuals in the program, enhancing skill development and recreational enjoyment, and encouraging participation in the program on a daily basis;
• Encouraging Direct Care Professional, the individual participants, families and guardians to become involved in the individuals activities and formal training;
• Facilitating community involvement as part of Day Habilitation, assuring access to events and activities enjoyed by those without IDD;
• Maintaining complete visual schedules for individuals in the program, and assuring effective communication with individuals and families in regards to Day Habilitation activities;
• Providing direct services to individuals in the program, including providing transportation to and from the Day habilitation program, as needed;
• Assists the Program Director in actively marketing the Day Habilitation program and promoting a positive profile within the community.

Knowledge, Skills, and Essential Functions
- Excellent written communication and computer keyboarding for inputting clinical documentation
- Excellent verbal and telephone communication with consumers, staff, and community providers
- Time management required to meet service goals
- Constant travel to provide services for clients, including regional travel about 50% of time and 50% local travel
- Stooping, bending, sitting, standing, and keyboarding for prolonged periods, lifting up to 30 lbs.
- Ability to be CPR and PMAB certified
- Valid driver's license, liability insurance, and reliable transportation required
Education and Experience
- Bachelor's degree in social work, psychology or related field Required
- 1 year of experience in case management and/or IDD preferred
- Direct experience with a family member with IDD can substitute for the degree requirement
Hours: M-F 7:30AM-4:30PM with other as needed
Salary: $30,000-$36,172 Annual DOQ
