The person in this position will be the front door to eligibility for IDD services and will maximize service capacity through consistent application of eligibility guidelines and will assure adherence to authority contract requirements and TAC rules governing IDD general revenue service eligibility.  The person in this position will explain IDD services available to people who are eligible, will outline the eligibility process, and will explain the support information needed to begin the eligibility process, including completing consents to gather information from physicians, school districts, psychologists and others as necessary. The person in this position will be required to have extensive information about community resources and will be the person responsible for working with other social service agencies regarding resource and information fairs and other community events.  The person in this position will be the center contact for and will participate in CRCG meetings across the six counties.   

EXAMPLES OF WORK PERFORMED:
  • Taking calls from, providing information to, and answering questions of people seeking information about IDD services and/or asking for the process of determining eligibility for IDD services;
  • Being familiar with the IDD services and supports available in the GR, HCS, TxHmL, ICF and CFC programs, in order to talk fluently and clearly about these services to people calling about IDD eligibility;
  • Completing intake packets to send to people calling for eligibility (or to provide to people who come in with questions about services), including relevant consent forms for the purpose of gathering documentation that supports eligibility for services;
  • Communicate routinely and clearly with diagnosticians so that they are aware of their testing schedules and have the information they need about people they are testing prior to the testing date; 
  • Coordinate the eligibility appointment for each individual so that they experience a seamless process;Keep up to date statistics about eligibility, including number of calls for services received daily, number of people actively in the pipeline for an eligibility determination, and number of people actually scheduled for eligibility determination.
Knowledge, Skills, and Essential Functions
  • Excellent written communication  and computer keyboarding for inputting clinical documentation
  • Excellent verbal and telephone communication with consumers, staff, and community providers
  • Time management required to meet service goals
  • Constant travel to provide services for clients, including regional travel up to 25% of time
  • Stooping, bending, sitting, standing, and keyboarding for prolonged periods, lifting up to 30 lbs.
  • Ability to be CPR and PMAB certified
  • Valid driver's license, liability insurance, and reliable transportation required
Education and Experience
  • Bachelor's degree in social work, psychology or related field Required
  • 1 year of experience working with people with IDD Required
Hours: M-F 8AM-5PM with other as needed

Salary:  $31,851- $35.036 DOQ
