A Wraparound Case Manager provides intensive evaluation, consultation and intervention with youth and families, within Heart of Texas Region MHMR catchment area, who are participating in wraparound services and/or YES waiver. Also participates in conducting psychosocial programming with this population as part of a clinical team.
 

EXAMPLES OF WORK PERFORMED:

  • Works with youth and their families in mental health, home or office settings
  • Provides case management and some skills training with children and families
  • Performs assessment and psychosocial evaluations and makes treatment recommendations
  • Refers families of clients to appropriate social welfare and health agencies to obtain assistance
  • Conducts family interview with every family to create a family story
  • Develops a crisis/safety plan with the family and updates as needed
  • Helps identify other community and individual supports that can be used by the family to achieve their care plan
  • Creates a Child and Family team for each youth and family
  • Is the primary contact person for each family in the wraparound system on their caseload and will be required to develop a plan of care for that individual family
  • Regular attendance at Family team meetings
  • Must complete Wraparound training
  • Must comply with all required documentation and data collection to support the program’s implementation and evaluation strategies
  • Other work as required
 

Knowledge, Skills, and Essential Functions

  • Excellent written communication  and computer keyboarding for inputting clinical documentation
  • Excellent verbal communication with clients, staff, and community providers
  • Time management and organization required to meet service goals
  • Local travel to client homes, school, jail, and other community settings which may include transporting clients
  • Stooping, bending, sitting, standing, keyboarding for prolonged periods, and lifting up to 15 lbs.
  • Ability to be CPR and PMAB certified
  • Valid driver's license, liability insurance, and reliable transportation required
 

Education and Experience

  • Bachelor's degree in social work, psychology or related field with ability to obtain Qualified Mental Health Professional credentialing
  • 1 year of case management experience preferred
  • Experience working with children and adolescents in a mental health setting preferred

Hours: M-F 8AM-5PM

Salary: $35,000-$37,450 DOQ
