Under the direction and supervision of the Veterans One Stop Program Director, provides services to veterans and their family members including assessment, treatment planning, case coordination, crisis intervention and referrals. Caseworker will maintain required documentation per community standards for Mental Health Community Services, as well as client and program records in accordance with applicable standards and regulations, grant requirements, etc.

Caseworker will work under direction and supervision of the Veterans One Stop Program Director with limited latitude for the use of initiative and independent judgment in carrying out details of the work.

  • Assists clients with navigating systems
  • Offer referrals for resources
  • Provide advocacy with delivery of systems
  • Provide continuity of care ensuring that client needs are met
  • Schedule individual and group sessions as needed
  • Builds relationships with local community agencies for referral purposes
  • Provides crisis counseling and supports as deemed necessary
  • Coordinates referrals to community-based providers
  • Prepares progress notes and other required documentation.
Knowledge, Skills, and Abilities
  • Excellent written communication  and basic computer knowledge for treatment planning and inputting clinical documentation
  • Excellent verbal communication and interpersonal relations with consumers, staff, and community providers
  • Time management and organization required to meet service goals
  • Local travel (15%) and regional travel (15%)  
  • Stooping, bending, sitting, standing, lifting up to 30 lbs., and keyboarding for prolonged periods
  • Ability to be CPR and PMAB certified
  • Valid driver's license, liability insurance, and reliable transportation required
Education and Experience
  • Bachelor's or Master's in social work, psychology or related field with ability to obtain Qualified Mental Health Professional credentialing
  • 1 year of case management experience Preferred
  • Veteran or Veteran family member Preferred
Hours: M-F 8AM-5PM with other as needed

Salary: $35,000-$41,195 ANNUAL DOQ
