Performs assigned case management services for Adult Mental Health Housing Services as well as a limited number of individuals who are not BHN consumers.  The case manager will assist consumers who are transitioning out housing instability by providing specialized case management and skills training while also assisting consumer in undertaking the goals on their treatment plan. 



  • Provides rehabilitation skills training as deemed necessary in accordance with Treatment Plan;
  • Provides case management specialized towards transitioning out of housing instability, focusing on budgeting, increasing income, landlord mediation, advocacy for consumers in disputes and independent living skills;
  • Interviews or corresponds with employment agencies, health and social welfare agencies, law enforcement officials, schools, and others to obtain significant background information which is needed for planning for the care of consumers;
  • Works with psychiatrists, other physicians, and other medical disciplines to assist in the rehabilitation program of consumers and in the training, care, and treatment program of consumers;
  • Develops job placements through contacts with community providers;
  • Attends staff meetings and participates with other professional staff team members in the planning of the ongoing care and disposition of consumer;
  • Prepares progress notes and other required documentation.

Knowledge, Skills, and Essential Functions

  • Excellent written communication  and basic computer knowledge for inputting clinical documentation
  • Excellent verbal communication with consumers, staff, and community providers
  • Time management required to meet service goals
  • Local travel to local jail, consumer homes and other community settings
  • Lift up to 30 lbs.
  • Stooping, bending, sitting, standing, and keyboarding for prolonged periods
  • Ability to be CPR and PMAB certified
  • Valid driver's license, liability insurance, and reliable transportation required

Education and Experience

  • Bachelor's or Master's in social work, psychology or related field with ability to obtain Qualified Mental Health Professional credentialing
  • 1 years of case management or criminal justice experience preferred

Hours: M-F 8AM-5PM with other as needed

Salary: $35,000-$41,195 ANNUAL DOQ
