The person in this position will be responsible for the completion of PASRR Level I and PASRR Level II Evaluations for people admitting to or residing in Nursing Facilities who are identified as potentially having Mental Illness or an Intellectual and/or Developmental Disability. These evaluations require face to face interviews with the individuals, their family members, Nursing Facility staff and others who know the individual, as well as research of existing medical records, for comprehensive discovery of pertinent information to accurately complete the evaluations. Responsibilities include active participation in initial interdisciplinary meetings for people eligible for PASRR services, completion of the Community Living Options process for people with IDD, follow-up for individuals with MI who refuse PASRR services, provision of case management or psychosocial rehabilitative training and other activities mandated by the HHSC contract or other rules relevant to the PASRR process.
EXAMPLES OF WORK PERFORMED:
- Complete PASRR Level I and Level II Evaluations;
- Provide face to face interviews of individuals, family members, NF staff and others in order to gather information pertinent to completion of the evaluations;
- Data entry of the completed evaluations into the TMHP portal;
- Attendance at the initial IDT meeting following the PASRR evaluation in order to discuss the specialized services recommended for the individual in the PASRR Evaluation and attendance to annual IDT meetings for those individuals who have refused services;
- For individuals with MI, completion of the ANSA to determine a service package appropriate for the individual’s needs;
- For individuals with IDD, completion of the Community Living Options process during the information-gathering phase of PE completion;
- Attendance at the annual IDT meetings for individuals who have previously refused PASRR services.
- Provision of case management and or psychosocial rehabilitative training as deemed necessary and in accordance with the individual’s treatment plan.
Knowledge, Skills, and Essential Functions
- Excellent written communication and computer keyboarding for inputting clinical documentation
- Excellent verbal and telephone communication with consumers, staff, and community providers
- Time management required to meet service goals
- Constant travel to provide services for clients, including regional travel up to 25% of time
- Stooping, bending, sitting, standing, and keyboarding for prolonged periods, lifting up to 30 lbs.
- Ability to be CPR and SAMA certified
- Valid driver's license, liability insurance, and reliable transportation required
- Bachelor's degree in social work, psychology, Human Service or related field Required
- 1 year of experience working with people with IDD Preferred
- 1 year Case Management experience Preferred
Hours: M-F 8AM-5PM with other as needed
Salary: $45,000-$49,500 for Bachelors level / $48,150-$52,965 for Masters level
