We are looking for an Assistant Director of Nursing to join our Queens office. Open to Full time or Part-time Hours. 

  • Assists Director of Nursing (DON) in provision of appropriate utilization of services, and compliance with regulations and Company policies.
  •  Assists DON in assuring that there is an appropriate match between patient / client needs and the skills and abilities of the care team assigned of each case.  
  •   Assists DON in management and supervision of a team of professional and paraprofessional employees accountable for providing competent care and achieving optimal patient/client outcomes.

Reports to the Branch Manager for non-clinical matters and the Director of Nursing on all clinical matters.

Responsibilities and Duties
  1. Reviews paperwork submitted via SPoC by Registered Nurse (RN) to ensure proper documentation. Educates RN in SPoC documentation and navigation software, if necessary.
  2. Schedules RNs’ visits to patient’s/client’s home. Ensures permanent aide supervisions, Resumption of Care (ROC), and Start of Care (SOC) are scheduled and done in a timely manner.
  3.  Ensures state and federal home care guidelines and Able Health Care policies and procedures are followed by the clinical staff to ensure compliance.
  4.  Addresses employees who are not meeting expectations and explains their responsibilities and duties and remind the employee that he/she are part of a team of caregivers. Perform necessary disciplinary action after warnings have been administrated.
  5.   Keeps records of all Recertification of patient’s/client’s medical services and SOC visits. Ensures that notices of these visits are submitted to the patient’s/client’s physician.
  6.  Resolves emergencies and other related needs of the patient/client or patient’s/client’s family, as it pertains to patient’s/client’s care.
  7.   Collaborates with the service coordinators on scheduling activities to guarantee consistency of caregiver assignments and clinical coordination of the services being provided.
  8.  Collaborates with the patient’s/client’s physicians, case managers and pharmacies to ensure quality patient care.
  9.  Schedules conferences with home health aides, either by telephone or in-person, to ensure policies and procedures are followed and to maintain home health aide compliance.  Keeps records of all conferences.
  10. Assists DON with Oasis review and entry
  11. Assists DON in the records documentation process to ensure that the patient’s/client’s clinical record meets legal and regulatory requirements, facilitates care, enhances the continuity of care, helps coordinate treatment and evaluation of the patient/client, and establishes medical necessity.
  12. Participates in agency’s Quality Assessment and Performance Improvement Program (QAPI) as it pertains to clinical performance and patient care.
  13.  Other duties as needed.

Education and Experience
Nursing Degree
NYS Registered Nurse license
Annual Physical within one year
PPD or Quantiferon within one year or chest x-ray within five years
MMR labs
COVID vaccine proof
