Position Objective 
  • Provides medical oversight for clients in their homes- including but not limited to assessments, obtaining MD orders, monthly contacts and maintaining an appropriate Plan of Care. Responsible for coordination, and supervision of the agency’s paraprofessional staff.   
  • Conduct reassessment of each patient on a regular basis; maintain complete and accurate clinical documentation;  
  • Ensure compliance with NYS HHA & PCA training requirements  
  • Teach classes and provide supervision of trainees during clinical training. 
  • Administer certifications for the HHA on the New York State registry.  
  • Conducts pre/post-employment skills and assessment activities.  
  • Perform competency evaluations of caregivers  
  • Supervise and serve as a resource to all home care staff

Position Requirements
  • The candidate for this position must be a Registered Nurse, preferably bachelor’s, prepared with licensure in New York State and acute or homecare experience with the last two years. Valid NYS licensure   
  • Knowledge of governmental home agency regulations, Medicare/Medicaid (as required) regulations, and Company policies are required   
  • Good organization and able to communication with students at all levels  
  • Self-motivation, assertiveness and independent decision-making skills are essential  
  • Bilingual English and Spanish, a plus

Pay
  • Salary range: $85,000 - $100,000 annually  
  • Leave time includes Paid Time Off, Sick and paid holidays

Work setting
  • Field visits; In-Office training
