Able Health Care Service is offering you a great opportunity to join our team. This is our 46th year in business. We are looking for a self-motivated Community Liaison who is driven and excels at developing new contacts. The Community Liaison will promote Able within the community through education and collaboration.

Responsibilities
  • Implement an Able Health Care Service education strategy, developing brand awareness, generating inbound traffic, and encouraging service/product adoption.
  • Maintain a solid understanding of the community by monitoring all aspects of the Home Health landscape that will impact the business, and recommending actions and contingency plans to maximize opportunities.
  • Networks on a person-to-person level in the healthcare community, to gain and maintain the trust of healthcare professionals.
  • Meets with families and individuals who need our services and facilitates the intake process by learning our client's needs to match them with compatible caregivers.
  • Works closely with our recruiting, scheduling, and intake departments, to provide the best possible caregivers for our clients.
  • Educates managed care organizations, networks, hospitals, doctors, assisted living & dialysis centers, and other external healthcare professionals regarding Agency programs, products, and services.
  • Attends job fairs, trade conferences, and other community outreach initiatives to educate on Agency services.
  • Assists with the development of social media, letter writing campaigns and proposals.
  • Work collaboratively with Contracting and Managed Care teams to identify/ execute new business opportunities.  Work with business partners to inform and educate the public about available services, eligibility, and costs.
  • Other duties as needed.

Qualifications
  • Degree in Business, Marketing, Public Administration, Health Administration, Social Work or related field, or an equivalent combination of education and experience.
  • Must understand and able to explain enrollment process to potential members. 
  • Five years’ work experience in healthcare, preferably in a community health, managed care plan, or home health care setting.
  • Basic knowledge of l home health agency regulations, Medicare/Medicaid regulations, Joint Commission Standards and Performance Improvement standards.
  • Effective communication skills. Bilingual (Spanish-English) skills required.

Pay
  • Base salary range: $55,000 – 65,000 annually plus bonus incentive 
  • Leave time includes Paid Time Off, Sick and paid holidays
