FIELD NURSES NEEDED for Home Care Agency

Job description:

Able Health Care Service is a leading home care provider that offers Home Health Aide/Personal Care Aide services to patients in need.

Our home care workers support and assist patients with carrying out activities of daily living. Able Health Care has been in business for 48 years and highly values and appreciates our clinical team.

We are seeking to hire F/T or P/T (Per Diem) RNs to visit patients homes to perform patient assessments and Home Health Aide supervision.

Field RN duties include

· Assess patients at the start of care to develop an individualized plan of care tailored to each patients needs and goals.

· Provide patient education and support to promote self-management.

· Provide Supervision of Able’s Home Health Aides, ensure they understand their role, tasks and establish competency to carry out such tasks.

· Reassess patients at certain time periods to determine any change of patient health status as well as supervise the Home Health Aide to assure the Aide is providing proper care to the patient.

· Document any/all status changes as per agency policy/procedures.

Qualifications

Valid Registered Nurse (RN) license in the state of New York

New RN Graduates welcome as Able will provide support and training.

Compensation

Full Time: $95K - $125K annually

Per Diem: $85 - $120 per visit based on service type.

  • RN Pad: 1 year (Preferred)

License/Certification:

  • RN (Required)
