The Program Manager – Harm Reduction Program (PM) will provide strong leadership to ensure that the agency operates a safe and secure system of housing that is outcome and performance driven and that staff are focused on maintaining permanent housing for participants. The PM is responsible for leading a collaborative effort to house the most vulnerable and chronically homeless living with HIV/AIDs. With the support of and in partnership with the participants of the Housing Opportunities with Person With Aids (HOPWA) funded agencies and the agency Harm Reduction Team, the PM will ensure a successful process for tracking, housing, and reporting the outcomes of the initiative.
Essential Duties & Responsibilities
  1. Cultivate continued collaboration among partners including Homeless Health Care LA, Aids for Aids, LACADA and other HOPWA-funded agencies and housing providers
  2. Expand, establish, and maintain effective working relationships with the community, collaborative partners, and participants to ensure a comprehensive continuum of outreach and support services for persons with HIV/AIDS
  3. Ensure seamless continuity of care and housing services between agency resources and partners agencies
  4. Ensure that services to participants reflect agency community model, which emphasizes customer choice, harm reduction, and psycho-social rehabilitation
  5. Report program outcomes and provide deliverables to partners and the community at large
  6. Oversee housing coordination, placement, and ongoing services for individuals in the Harm Reduction Program
  7. Coordinate trainings, community events, community collaborations, and outreach
  8. Provide individual and team supervision, support, and performance evaluations for assigned staff
  9. Assist in recruitment, selection, and training, of assigned staff
  10. Identify and lead the coordination of housing-related emergency financial assistance, medical and mental health treatment, education, vocational training, and other services
  11. Ensure housing performance standards of partners and funders are created for all direct service staff and ensure productivity goals and outcomes are achieved including a minimum of 80% housing retention rate
  12. Provide input, evaluation, and updates regarding programs as requested
  13. In coordination with supervisor, provide budgetary oversight of program administration and ensure program revenue and expenses are within adopted annual budget
  14. Complete required reporting (e.g., Exym, LAHD, and HMIS) as outlined by various funders
  15. Manage and maintain the LAHD HIMS data entry system to track the registry and housing placements achieved through the Program
  16. Ensure all participant paperwork, data collection, and maintenance is complete, timely, accurate, and current in agency records and electronic databases
  1. Bachelor’s degree in related field (Social Work, Psychology, or other Human Services) and four years’ full-time experience working with the homeless, preferably those living with mental illness and / or substance addictions; or an equivalent combination of education and experience
  2. Knowledge of other social service providers
  3. Ability to communicate and interact with clients in a culturally competent manner
  4. Detail oriented with excellent time management, organizational, written, verbal, interpersonal, presentation, teambuilding and computer skills
  5. Valid CA Driver’s license, reliable car, auto insurance, and an acceptable driving record
  6. Able to work effectively with a wide range of constituencies
  7. Able to work with and lead community groups and volunteers
  8. Self-starter with the ability to stay ahead of the curve and thrive in fast-paced work environment
  9. Strong understanding of and belief in the Harm Reduction philosophy, and able to apply it with participants
Preferred Qualifications
  1. Masters in Social Work, Psychology, or other Human Services field
  2. Two years’ supervisory experience
  3. Knowledge and experience in chemical dependency and substance abuse treatment
  4. Prior advocacy or academic experience in housing and/or homeless services
  5. Bilingual in English/Spanish
  6. Minimum 30 WPM typing speed
