Job Coach
Position Summary                                       
The Community Support Coach position is responsible to ensure individuals with disabilities are receiving on-the-job support.  The Coach  will  teach,  assist and explore opportunities for individuals to succeed.

Essential Job Functions
  • Provide one-on-one support to participants on-the-job to ensure workplace success.
  • Maintain a positive rapport with area businesses and employees.
  • Develop an individualized plan for job coach fading to help a participant work toward self-sufficiency and success on the job.
  • Work cooperatively with other LCS department personnel and supervisor.   Communicate daily schedules.
  • Identify and implement adaptive devices to help participants be successful on the job.
  • Complete monthly DVR and/or long -term care billing along with monthly progress reports.
  • Assist in the development of participant goals.
  • Demonstrate a high degree of patience, reliability, and sensitivity toward others during coaching.
High School diploma or equivalent is required.  Two (2) year degree in Human Services is preferred.
At least two (2) years of experience working with people with disabilities.
Must possess a valid driver’s license, car insurance and active mobile phone.
Excellent time management skills, and the ability to work independently.
Must have a flexible schedule to accommodate possible nights, weekends, and holiday job coaching needs.
