Duties/Responsibilities:  Kentucky Moms MATR Targeted Case Manager’s primary responsibility is that of working with pregnancy women and 60 day postpartum mothers who have moderate to severe substance use dependence.  This includes case finding, education, and assessing the person in an individual setting and making referrals to other helping agencies.  Targeted Case Manager will coordinate relationships among other human services agencies, consult the courts, the Department for Community-Based Services (DCBS), domestic violence shelters, law enforcement, and the public in general.  Targeted Case Manager will be the initial contact worker for referrals received from local health departments.  All other duties outlined in current job description. 
 
Skills/Knowledge: Must successfully complete DBH approved curriculum of targeted case management training within six (6) months of employment and maintain continuing education hours for certification.  Must have at least one (1) year of full-time employment experience working directly with adults in a human service setting.
