Responsibilities include:
  • Ensuring that the medically related emotional and social needs of the resident are met and maintained.
  • Developing assessments and written plans of care to identify the needs of each resident.
  • Assessing the resident's psychosocial and emotional needs.
  • Effectively communicating information concerning a resident’s condition with other departments and government agencies.
  • Participating in resident care planning conferences.
  • Participating in the resident discharge planning, providing information to residents and families of programs available.
  • Serving as a liaison with community agencies to ensure needed ancillary services are available and provided within requested timeframes.
  • Making referrals and documenting outcomes.
  • A strong commitment to maintaining the confidentiality of all resident care information in accordance with HIPAA Guidelines. 

Qualifications:
  • Bachelor’s degree is required; focused in Social Work or other Healthcare related fields required.
  • Recent experience in a Social Services role in a nursing home/hospital setting or some other Social Worker role required.
  • Ability to work in a fast-paced work environment.
  • May occasionally to work weekends. 
  • Excellent communication and customer service skills.
  • Strong computer skills required due to this facility working solely in an Electronic Medical Records environment.
  • The ability to pass a background check is required of all Maryville Employees.
Physical requirements:
  • Must be able to lift 25 pounds at times.
  • Regularly required to use hands and fingers to handle, feel, or operate objects, tools or controls and reach with hands and and arms; frequently required to stand walk, talk, sit, and hear' occasionally required to climb, balane, stoop, kneel, crouch, reach above shoulder height, reach below the waist, or lift as needed throughout the work day. Proper lifting techniques are required.
