Maryville is seeking a full-time Social Worker to join its well-respected team of dedicated health-care professionals. Responsibilities include:
  • ensuring that the medically related emotional and social needs of the resident are met and maintained;
  • developing assessments and written plans of care to identify the needs of each resident; assessing the resident's psychosocial and emotional needs;
  • effectively communicating information concerning a resident’s condition with other departments and government agencies;
  • participating in resident care planning conferences;
  • participating in the resident discharge planning, providing information to residents and families of programs available;
  • serving as a liaison with community agencies to ensure needed ancillary services are available and provided within requested timeframes;
  • making referrals and documenting outcomes.

  • Bachelor’s degree or equivalent experience is required; focus in Social Work or Healthcare administration strongly desired;
  • Recent experience in a Social Services role in a nursing home/hospital setting or some other Social Worker role is desired;
  • Ability to work in a fast-paced work environment;
  • Excellent communication and customer services skills required;
  • Strong computer skills required due to this facility working solely in an Electronic Medical Records environment.
