Mālama I Ke Ola Health Center (Community Clinic of Maui) is a Federally Qualified Health Center (FQHC) and a fully integrated behavioral health, dental, and primary care system. Our nearly 100 professionals and providers serve the people of Maui at three clinics—two locations in Wailuku and one in Lahaina. We have behavioral health providers who partner with the primary care clinicians to assist in a vast number of activities including warm handoffs for acute behavioral health crisis management, substance abuse counseling, tobacco cessation counseling, chronic disease self management counseling, and long-term individual and family therapy. In 2015, we served over 11,000 patients of which 97% are living at or below 200% of the Federal poverty level.  The Community Health Program Coordinator contributes to meeting the comprehensive needs of the patient from social determinants of health to direct medical services. Under the Primary Care Medical Home (PCMH) model, the Program Coordinator works in conjunction with the clinical team in the specific populations to place services and contribute to patient outreach, education, navigation, social support, medical follow-up needs, and advocacy, to achieve positive outcome for chronic disease management and risk factor reduction. The Program Coordinator will implement the lifestyle program and supervise daily operations related to the lifestyle program.
 
Essential Duties and Functions:
 
  • Establishes relationships with public health, physician, payer communities, and other referral networks to enhance awareness of and referrals to the lifestyle program.
  • Responds to inquiries about the lifestyle program from the general public and members of the public health, physician, health care provider, and payer communities.
  • Engages senior leaders within the organization to be ambassadors and advocates for the lifestyle program in the public health, physician, health care provider, and payer communities.
  • Acts as spokesperson for the lifestyle program to the press and media.
  • Recruits and supervises Community Health Workers.
  • Organizes a master schedule of the lifestyle program classes offered by the health clinic.
  • Ensures adequate publicity for and marketing of the lifestyle program.
  • Assists with retention and commitment of lifestyle program participants.
  • Reviews data from the lifestyle program to ensure it meets quality and performance standards.
  • Ensures compliance with all applicable laws and regulations, including those governing privacy and data security.
 
Education:
 
  • High school  Diploma or equivalent
  • Trained as a lifestyle coach desirable however training will also be given
 
Experience:
 
  • Minimum two years’ experience as a program coordinator working in a community-based setting
 
Skills:
 
  • Written and oral fluency in English required. Experience working in the multi-cultural setting unique to Maui
  • Knowledge of some medical terminology preferred.
  • Basic computer skills.
  • Understand the community served – community connectedness.
  • Ability to initiate and maintain positive working relationships with staff and other organizations.
  • Good communication skills, such as listening well, and using language appropriately.
  • Ability and willingness to provide emotional support, encouragement and motivation to patients.
