Community Clinic of Maui, Inc. dba Malama I Ke Ola Health Center is a Federally Qualified Health Center (FQHC) and a fully integrated behavioral health, dental, and primary care system. Primary Function
The Family Nurse Practitioner/APRN is responsible for providing outpatient diagnostic, treatment, and preventive health care services; management of chronic illness; utilizing appropriate referrals and consultation to staff physician and community specialists; health education to patients. This position is a school-year position specific to primary and secondary public schools on Maui. This role will utilize the NP responsibilities and apply to the students and adults within the school as well as include some assignments at our community clinic in Wailuku, Maui, Hawaii, and satellite clinics on Maui.
Essential Duties and Functions:
1. Provides comprehensive health care services to all patients, including but not limited to, well-child care, acute care and chronic care.
2. Provides patient and family education to promote wellness, to prevent health problems, and to maintain current health status as well as intervene in acute or chronic illness states.
3 Coordinates client care with other community professionals and organizations for optimal care.
4. Works collaboratively with supervising pediatrician, chief medical officer, clinic management team and other nurse practitioners.
5. Works collaboratively with school and school district to determine student needs and align clinic care and offerings accordingly.
6. Participates in the chart audit process as specified in the quality improvement program.
7. Completes all charts in electronic medical record database within 24 hours of visit.
8. Participates in planning of clinic provider schedule to ensure clinic coverage.
9. Provides input into decision-making process by attending provider and clinic staff meetings.
10. Assists in developing goals and objectives for the program including quality improvement.
11. Participates in problem-solving of issues and concerns affecting the pediatric/adolescent program.
12. Maintains current continuing educational hours, certification, and licensure.
13. Participates in call coverage 4-6 weeks out of the year to handle more urgent needs of clinic patients.
Qualifications:
The ideal candidate will have APRN or FNP from a recognized program, evidence of continuing education, competencies with providing services to a diverse patient population as an APRN or NP of at least one year, and verifiable work background in treating all individuals with respect and aloha. Master in Science or Public Health preferred. Previous school-based program experience highly desired. This position reports to the Chief Medical Officer at our Wailuku clinic. Full-time during the school year with summer assignment options at our Maui health centers, Exempt.

- Current CPR, negative TB within past 12 months, and APRN or NP licensure/NED registration in the State of Hawaii required.
- Must be fully vaccinated against COVID-19 or provide an approved medical or religious exemption.
