Community Clinic of Maui, Inc. dba Malama I Ke Ola Health Center is a Federally Qualified Health Center (FQHC) and a fully integrated behavioral health, dental, and primary care system. The Quality Manager reports to the Clinical Operations Director to support our quality assurance system and team that promotes customer satisfaction and that positively impacts the financial performance of Mālama I Ke Ola Health Center. The Quality Manger is one member of a team to champion continuous quality improvement efforts in the organization.
In conjunction with the Clinical Operations team, this position provides oversight, data-synthesis, coordination, and direction to impact performance on key metrics of quality and safety and achieve or maintain programmatic excellence standards. Collaborates with the medical staff and other key professionals to design comprehensive programs to create a culture of safety, proactively identify risk, and reduce clinical variation to assure evidence-based care delivery and prevent patient harm. This team facilitates organization-wide learning on the part of leaders, clinicians and staff; and facilitates individual improvements required for the safest possible health care.

ESSENTIAL DUTIES AND FUNCTIONS :
- TEAMWORK
- Participates in the clinic policies and procedures with the Clinical Operations team.
- Collects, organizes and summarizes data for performance measurement, internal, state, and federal reporting and decision-making.
- Works with Population Health/HIT on patient access and engagement as it relates to the Quality Program.
- Works with Clinical Operations team to review patient feedback.
- Acts as a resource for process improvement projects.
- Acts as a resource regarding compliance with PCMH/MU standards.
- Educates and trains employees as to their impact to continuous quality improvement.
- PROBLEM SOLVING AND ANALYSIS
- Facilitates root cause analysis for sentinel events.
- Routinely gathers, analyzes, and disseminates information regarding patient feedback, incidents, and sentinel events.
- COMMUNICATION
- Meets regularly with the clinical leaders and managers to address issues and needs.
- Facilitates communication of organization wide quality goals at the site level with providers and operations.
- Liaison to, or member of, organizational committees as appropriate.
- Represent Mālama I Ke Ola externally on committees and/or collaborations as designated by the Clinical Operations Director, Chief Medical Officer, or Chief Executive Officer.
- Works with Clinical Operations Director and Senior Leadership on the annual Quality Plan, the selection and measurement of aims, and develop data reports. Effectively coordinates this process and provides support for related QI projects.
- Maintains and communicates dashboard results to organization throughout the year in collaboration with the Clinical Operations Director and Health Information Data Analyst.
- Routinely coordinates PCMH/MU applications with the Clinical Operations Director and Health Information Data Analyst. .

- WORKPLACE COMPUTERS AND EQUIPMENT
- Acts as data champion for the clinic by mastering EMR and related report building and project software.
- Validate data from generated reports and uses data to monitor quality standards, in conjunction with the Health Information Data Analyst.
- Monitors and analyzes data from various sources (e.g. patient feedback, PSATs, external reviews, incident reports) to determine areas that require improvement.
- CHECKING, EXAMINING, AND RECORDING
- Maintains appropriate documentation of all work activities.
- Performs and/or coordinates audits of relevant and timely issues.
- COMPLIANCE, POLICIES, AND PROCEDURES
- Assists Clinical Operations Director with policy and procedures reviews and revision that pertain to operations efficiency, patient engagement, regulatory practices, HIPAA compliance, OSHA, HIPAA, Title X, Perinatal Support Services, staff TB program, and BPHC UDS and other required accrediting/regulatory/grant/federal programs.

Education:
- Associates degree preferred in a health-related field, continuous quality improvement methods, data analysis, and basic knowledge of medical terminology. High School/GED required.
- Two or more years of experience with continuous quality improvement programs, including the ability to follow, edit, and update job-related protocols, policies, and procedures. Experience in a health-related field preferred and will consider other fields with potential transfer skills to healthcare.
- Excellent verbal and written communication skills, including active listening.
- Ability to plan and organize projects.
- Must have ability to logically analyze and optimize patient schedules.
- Must possess leadership skills.
- Computer literate in word processing and spreadsheets. Knowledge/skills of basic computer operations, i.e. word processing, Microsoft Suite, with Excel proficiency, database, project management, and ability to learn our electronic medical records system.
- Other requirements:
TB Clearance within the past 12 months
Proof of COVID-19 Vaccination as required by federal mandate or ability to provide acceptable medical or religious exemption.
