The Rescue Mission of Trenton is seeking a dedicated and compassionate Care Coordinator – Homeless Assessment and Referral (HAR) to join our Emergency Shelter team. In this role, you will provide direct case management and support services to individuals experiencing homelessness, helping them overcome barriers, access critical resources, and take real steps toward stable housing and independence.

What You’ll Do
• Provide direct case management services to individuals experiencing homelessness
• Conduct assessments and develop individualized service and treatment plans
• Coordinate referrals for housing, behavioral health, substance use treatment, financial support, and justice-related services
• Work closely with internal teams and external agencies to ensure coordinated, wraparound care
• Support clients in overcoming barriers to housing and stability
• Offer grounding techniques, comfort measures, and choice-based supports for clients experiencing anxiety or environmental stressors
• Maintain accurate and timely clinical documentation in HMIS and client records
• Monitor progress and adjust service plans as needed
• Advocate for clients and contribute to improving program outcomes and service delivery
• Participate in team meetings, training, and ongoing professional development 

What We’re Looking For
• Bachelor’s degree in Psychology, Sociology, or a related human services field
• 1–2 years of case management or human services experience required (homeless services experience strongly preferred)
• Knowledge of local mental health and substance abuse treatment providers and community resources
• Understanding of fee-for-service models and community-based behavioral health systems
• Strong communication, critical thinking, and active listening skills
• Understanding of addiction, behavioral health, trauma-informed care, and housing instability
• Experience with clinical documentation and maintaining accurate client records
• Must have strong computer and typing skills, including accurate data entry and documentation
• Proficiency in Microsoft Office (Word, Excel) and HMIS
• Valid driver’s license required
• Ability to work in a collaborative, fast-paced, mission-driven environment

Schedule: Sunday, Tuesday, Thursday: 9:00 AM – 5:00 PM, Monday & Wednesday: 11:00 AM – 7:00 PM
Salary: $44,928 – $47,528
