Essential Duties and responsibilities include, but are not limited to the following:
- Guides prospective residents/patients and families through admissions, answering questions, and ensuring a positive experience.
- Reviews patient charts, gathers necessary clinical information.
- Performing pre-admission assessments to evaluate a patient's medical needs, medication lists, and required level of care.
- Completing all state and facility-required admission paperwork, including electronic medical records (EMR) as necessary.
- May perform nursing duties during admission, ensuring care standards are met.
- Works with clinical and administrative staff for seamless patient transitions.
- Assist other departments involved with the admissions process to accomplish a smooth, efficient admission process.
- Other duties as assigned by the Administrator.
Education and Experience:
- Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license in the state of practice (Oregon).
- One (1) years’ experience in health care, particularly in long-term care or home health admissions.
Required Skills/Abilities:
- Proficiency in Electronic Medical Records (EMR) systems.
- Strong clinical judgment and assessment skills.
- Excellent communication and conflict- resolution abilities for dealing with families and referral sources.
- Highly organized with the ability to manage high-volume paperwork and multitasking.
- Excellent customer service skills.
- Ability to read, write and speak English.
- Ability to exercise discretion in confidential matters.
- High degree of accuracy in data entry.
- Excellent attention to detail.
- Computer literacy: Work processing and spreadsheets.
- Public speaking skills necessary to interact in a variety of situations.
Preferred Skills and Experience:
- Two (2) or more years’ experience in health care, particularly in long-term care or home health admissions.
- Strong communication for interviewing families and resolving conflicts, high attention to detail for medical records, and the ability to work in a fast-paced environment.
Physical Requirement:
- Works in temperature-controlled office areas, but frequently moves through the entire facility, including patient rooms, therapy areas, and common lobbies.
- Characterized by moderate noise levels with frequent interruptions from phone calls, medical staff, and arriving families.
- Required to stand or walk for prolonged periods, especially when conducting facility tours or performing building rounds.
- Ability to lift, push, pull or move at least 50 pounds independently on occasion.
- May occasionally assist with patient transfers or evacuations, which can involve moving up to 200 pounds with assistance or equipment.
- Potential exposure to infectious diseases (e.g., Hepatitis B), chemical cleaning solutions, and medical waste, requiring strict adherence to HIPAA and infection control protocols.
- Requires regular bending, stooping, reaching, and kneeling.
