Performs mental health services for severe adult mentally ill clients.  Work involves conducting interviews and clinical assessments; site-crisis coverage for referrals and, provision of continuity of care to referred mental health individuals in crisis.  Works under moderate supervision of the Supervisor with limited latitude for the use of initiative and independent judgment in carrying out details of the work.


  •  Provides continuity of care services for individuals transitioning out of crisis
  • Develops comprehensive treatment plans and monitors progress
  • Coordinates referrals to community-based providers
  • Provides on-site crisis assessments and develops intervention plans
  • Provides crisis counseling and supports as deemed necessary
  • Prepares progress notes and other required documentation.

Knowledge, Skills, and Essential Functions

  • Excellent written communication  and computer keyboarding for inputting clinical documentation
  • Excellent verbal communication with consumers, staff, and community providers
  • Time management required to meet service goals
  • Local travel 30% to consumer homes, school, and other community settings
  • Stooping, bending, sitting, standing, keyboarding for prolonged periods, and lifting 15 lbs.
  • Ability to be CPR and PMAB certified
  • Valid driver's license, liability insurance, and reliable transportation required. 50% local travel

Education and Experience

  • Bachelor's or Master's in social work, psychology or related health services field
  • 1 year case management experience preferred 

Hours: M-F 8:00AM-5:00PM with some flexibility required to accommodate the school schedule

Salary:  $30,000-$36,172 Annually DOQ
