The person in this position will directly supervise the Service Coordination staff and Lead Service Coordinators for the HCS Waiver program.  The person in this position is responsible for assuring adherence to authority contract requirements, SC TAC rules, SC manuals, the LIDDA handbook and other mandates issued by HHSC that effect provision of Service Coordination to people in the HCS program. 

Example of Work Performed
  • Implementing policy and procedure as they relate to hiring, training and evaluating HCS Service Coordinators; including providing structured feedback and if necessary disciplinary action. Directing all Service Coordination activity in the HCS program.
  • Develop and maintain a cooperative working relationship between internal authority and private service providers, managed care organizations, and community entities. 
  • Developing and maintaining data in regards to service capacity in the HCS service coordination unit, including maintenance of up-to-date SC caseloads, and using data for future planning and development.
  • Facilitating transfers in the HCS Waiver, in and out of the Heart of Texas area.
  • Directing Quality Assurance activities regarding Service Coordination including Person Centered Discovery, development of outcomes for the PDPs, and comprehensive service monitoring. 
  • Provide effective adjustment to SC caseloads to address over-capacity in the unit or to address temporary vacancies in SC positions.
  • Reviewing Service Coordination Type A and B documentation and essential service provision reports in order to assure comprehensive, accurate and timely service data entry that maximizes service quality and enhances revenue collection of the SC unit.

Knowledge, Skills, and Essential Functions
  • Must exhibit an understanding of Service Coordination for Individuals with Intellectual Disabilities, and an understanding of the Person Centered process for developing individual plans and service outcomes. 
  • Must be familiar with HHSC and other policies and procedures as they relate to providing Service Coordination to people in the HCS waiver program.
  • Knowledge of community resources is essential. 
  • Must have problem solving and negotiation skills, as well as the ability to interact effectively with consumers, LARs, private service providers, managed care organizations, and other community entities.   
  • Excellent written communication  and computer skills in Microsoft Word and Excel
  • Excellent verbal communication plus supervisory skills
  • Time management required to meet service goals
  • Approximately 15% regional travel and 85% local travel
  • Stooping, bending, sitting, standing, and keyboarding for prolonged periods, and lifting up to 30 lbs.
  • Ability to be CPR and PMAB certified
  • Valid driver's license, liability insurance, and reliable transportation required
Education and Experience
  • Master's degree in social work, psychology or related field with ability to maintain Qualified Mental Health Professional credentialing
  • 2 years case management experience Required
  • 2 years IDD experience Preferred
  • 1 year in a supervisory experience Preferred
  • Extensive knowledge of Service Coordination in the HCS program Required

Hours: M-F 8AM-5PM

Salary: $43,000  annually
