Under the direction and supervision of the Crisis Services Director, develops a therapeutic environment for the crisis respite program. Works closely with agency staff as well as community stakeholders to facilitate respite services to individuals in crisis. Completes referral paperwork including treatment plans on each individual entering the crisis respite program. Ensures individual and groups are offered as well as structured activities. Works closely with The Wood Group staff to meet needs of individuals in the crisis respite program. Coordinates appropriate services with assigned caseworkers, MCOT, triage, etc as well as other staff involved in providing therapeutic services for individuals. Provides coverage for other units as needed.   

Works under moderate supervision of the Crisis Services Director with limited latitude for the use of initiative and independent judgment in carrying out details of the work.

EXAMPLES OF WORK PERFORMED:
                     
  • Provide structured activities for individuals in respite services
  • Ensure individual and group services are offered
  • Builds relationships with local community agencies for referral purposes
  • Provides crisis counseling and supports as deemed necessary
  • Coordinates referrals to community-based providers
  • Prepares progress notes and other required documentation.
  • Complete respite referral paperwork and treatment plan
  • Coordinates services with The Wood Group and case workers

 Knowledge, Skills, and Abilities
  • Excellent written communication  and basic computer knowledge for inputting clinical documentation
  • Excellent verbal communication with consumers, staff, and community providers
  • Time management required to meet service goals
  • Local travel to consumer homes and other community settings
  • Lift up to 30 lbs.
  • Stooping, bending, sitting, standing, and keyboarding for prolonged periods
  • Ability to be CPR and PMAB certified
  • Valid driver's license, liability insurance, and reliable transportation required
Education and Experience
  • Master's degree in a human service field Required
  • LPC, LCSW, LMFT full licensure Required
  • 1 years of case management experience preferred
  • Experience in leading groups and  engaging with community partners
Hours: M-F 8AM-5PM with other as needed

Salary: $52,000 Annual Salary
