Case Managers serve as the primary point of contact at Heart of Texas Behavioral Health Network for a caseload of adults diagnosed with serious mental illness. Duties involve assisting clients develop their personal recovery plan, completing clinical assessments to determine needs and measure progress, making referrals to other agencies, providing skills training to help clients achieve their personal goals by learning to better manage symptoms of their illness. Case Managers provide crisis intervention. Case Managers provide services in the office and in the field, helping ensure clients physical needs are being met and helping facilitate access to things that are important to success in their behavioral health treatment. To effectively perform duties, Case Managers need strong organizational skills, computer skills, and consistent ability to demonstrate compassion and collaboration with clients, families, staff within the agency and community partners.
Salary: $45,000-$49,500 for Bachelors level / $48,150-$52,965 for Masters level DOE
Depending on location a regional stipend of $5000 will be added
