The Housing Navigator will work closely with area Continuum of Care agencies to locate housing for homeless individuals.  Locating appropriate housing can be difficult.  Being low-income, homeless and disabled makes the process even more difficult.  The Housing Navigator will walk the consumer through the process of locating and applying for appropriate housing.  They will follow the consumer from the time they are approved for a supported housing program up until their lease signing.  The Housing Navigator will also act as liaison between area landlords and the Continuum of Care to develop relationships to increase the availability of affordable housing.  They will also respond to concerns a landlord may have with a consumer in one of the Continuum of Care housing programs. 


EXAMPLES OF WORK PERFORMED:
  • Develop relationships with area landlords to increase pool of affordable housing.
  • Help consumers locate appropriate housing.
  • Coordinate services and support with community partners to ensure a smooth transition from homelessness to housing.
  • Work with community partners to provide quick response and resolution to concerns a landlord may have with a consumer. 
  • Prepare progress notes and other required documentation.

KNOWLEDGE, SKILLS AND ABILITIES:
  • Excellent written communication and basic computer knowledge for inputting clinical documentation
  • Excellent verbal communication with consumers, staff, and community providers
  • Time management required to meet service goals
  • Local travel 40% and 20% Regional travel
  • Lift up to 30 lbs
  • Ability to be CPR and PMAB certified
  • Valid driver's license, liability insurance, and reliable transportation required
Education and Experience:
  • Bachelor’s (CWI) or Master’s Degree (CW II) in Psychology, Social Work or related field.
  • 1 year of case management preferred
  • Experience working with Behavior Health Population preferred
Hours: M-F 8AM-5PM with other as needed

Salary: $45,000-$49,500 for Bachelors level / $48,150-$52,965 for Masters level
