The person in this position will work closely with the HCS SC Supervisor to assist in the training of new service coordinators and the ongoing training of staff in the HCS service coordination unit.  The person in this position will also serve as the primary back-up to the HCS SC unit, which means taking over caseloads when SC positions are vacated or when an SC is on extended leave.  In that role, the Lead HCS SC is responsible for all duties of a regular SC, including developing Person-Directed Plans, coordinating the development of IDRCs and IPCs, and completing monitoring requirements and productivity standards in the SC position.  The person in this position must be able to effectively communicate with consumers/families, service providers, and staff in other community agencies. The person in this position will also serve as the Lead trainer of new staff in regards to the electronic health record.


  • Provide on-boarding to new Service Coordinators, including training on policies and procedures, waiver requirements, SC responsibilities, and providing opportunities for shadowing other SCs; 
  • Provide training to new Service Coordinators on effective use of the Electronic Health Record;
  • Serve as primary backup to the Service Coordinators in the unit, and maintain readiness to take over a caseload when a position is vacated or an SC is out on extended leave;
  • Provide quality assurance by conducting periodic clinical record reviews and follow-up, focusing on Person-Directed Plans and regular monitoring notes at the direction of the HCS SC Supervisor;
  • Assist SC staff in identifying new community resources for people in the waiver, and serve as the advisor to SC staff to troubleshoot issues that may arise between Service Coordination and Provider functions.
Knowledge, Skills, and Abilities:
  • Must have a firm understanding of the HCS waiver program and Service Coordination rules. 
  • Must be able to work effectively with other Service Coordinators. 
  • Must be able to effectively interact with consumers, families and service providers to maintain an adequate plan for each consumer on the caseloads. 
  • Must be able to consistently and in a timely manner produce documentation as required by the HCS SC rule and billing guidelines. 
  • Must be flexible in schedule yet organized to keep up with the demands of individual consumers and program requirements. 
  • Must be able to monitor the effectiveness of the services received by the consumers and communicate concerns to providers.

Service Coordination Salary:  Current Salary plus $2500 Lead  Stipend
