EXAMPLES OF WORK PERFORMED:
  • Provides short-term counseling and skills training services to patients and their families in rural integrated health settings
  • Provides care coordination, system navigation, crisis intervention, and community resource linkage and referrals for individuals served 
  • Work with interdisciplinary team to coordinate both behavioral and physical health care in rural primary care settings
  • Complete all documentation including treatment plans, treatment plan reviews, progress notes, discharge summaries and other relevant documents

Abilities, Skills, and Essential Functions

  • Excellent written communication  and basic computer knowledge for treatment planning and inputting clinical documentation
  • Excellent verbal communication and interpersonal relations with consumers, staff, and community providers
  • Time management and organization required to meet service goals
  • Local travel (20%) and regional travel (80%). 
  • Stooping, bending, sitting, standing, lifting up to 15 lbs., and keyboarding for prolonged periods
  • Ability to be CPR and PMAB certified
  • Valid driver's license, liability insurance, and reliable transportation required
Education and Experience
  • Master's in social work, psychology or related field with ability to obtain Qualified Mental Health Professional credentialing Required
  • LPC, LCSW, LMFT required OR currently under supervision for licensure Strongly Preferred
  • 1 years of case management experience Preferred
  • Understanding of Mental Illness and role of Care Coordination Preferred

Hours: M-F 8AM-5PM with other as needed

Salary: Master's $48,150-$52,965 Annual DOE 
with an additional Regional Stipend $5000

            Licensure $64,000 Annual with an additional Regional Stipend $5000
