Under the direction and supervision of the McLennan County Reintegration Program Manager, provides services to justice involved individuals and their family members including assessment, treatment planning, case coordination, crisis intervention and referrals. Caseworker will maintain required documentation per community standards for Mental Health Community Services, as well as client and program records in accordance with applicable standards and regulations, grant requirements, etc.

Caseworker will implement a client-centered case management approach, working with individuals who are currently incarcerated and/or have been released from custody, and who require immediate supports due to a lack of resources and opportunities in the current economy in order to support a successful reentry to the community. The caseworker will work closely with the Jail Diversion Team, the Caseworker will conduct intakes and assessments and provide direct assistance, information and referrals to the community; develop individual action plans that are outcomes based and provide continued support and follow-up to enhance individual success and substantiate outcomes reporting. The case worker will work closely with the judicial system. The Caseworker will also be responsible for data collection, casework documentation, and database entry along with summary reporting. Assistance to other members of the team will be provided as necessary.

EXAMPLES OF WORK PERFORMED:
                     
  • Assists clients with navigating systems of reentry
  • Offer and arrange referrals for resources with community-based providers
  • Completes assessments
  • Deliver pre-release life skills training
  • Provide effective continuity of care ensuring that client needs are met
  • Builds relationships with local community agencies for referral purposes
  • Builds relationships with individuals working in and for the judicial system
  • Provides crisis counseling and supports as deemed necessary
  • Prepares progress notes and other required documentation
  • Data collection and proper database entry for effective program outcome measures
  • Provides individual sessions
  • Facilitates group sessions
  • Teach class withing the jail up to three times a week
KNOWLEDGE, SKILLS, AND ABILITIES:
 
 
  • Knowledge of mental illness and emotional components of DSM-5, psychosocial assessments and crisis intervention.
  • Considerable ability to work cooperatively and effectively with professional personnel; to exercise tact, initiate, and good judgment in dealing with clients and relatives; to prepare reports and make recommendations for administrative use; to speak and write effectively; to relate to people and to interact with consumers.
  • Participates actively as a team member in supporting clients and the program as needed.
  • Must be comfortable working with inmates within the McLennan County Jail.
  • Must have basic computer skills

Experience and Training

Requires successful experience in providing mental health services for persons, a working knowledge of DSM-5, psychosocial assessments and crisis intervention.  LCDC or LCDC-Intern Level II preferred. Requires excellent communication skills.

Education/License

Graduation from an accredited college or university with a bachelor’s or master's degree with a major in social work, psychology, human services, or other related field required. Prefer prior experience in the field.

Must have current valid drivers license and current and continued proof of liability insurance.

Salary: $45,000-$52,965 DOE

Hours: Monday-Friday 8am-5pm
