A Case Manager provides evaluation, consultation and intervention with youth and families within the community.  Also participates in conducting psychosocial programming with this population as part of a clinical team.

EXAMPLES OF WORK PERFORMED:
 
  • Works with youth and their families in school, mental health, community, home or office settings.
  • Works with community and family for the purposes of creating and maintaining individualized service arrays for families.
  • Provides case management and some skills training with children and families.
  • Makes assessment and psychosocial evaluations and recommendations.
  • Refers families of clients to appropriate social welfare and health agencies to obtain assistance.
  • Performs Related Work as Required.    

Experience and Training:                    
  • Experience as set forth under Education. 
  • Must have current valid driver’s license and current and continued proof of liability insurance.
  • Case management experience preferred.

Education:
Bachelor’s or Master's Degree in Psychology, Social Work or related field.

KNOWLEDGE, SKILLS AND ABILITIES:    
  • Knowledge of emotional problems as well as rehabilitation/skills training and treatment approaches. 
  • Must have excellent verbal/written communication and knowledge to enter clinical documentation.
  • Ability to establish and maintain positive working relationships with clients and colleagues. 
  • Skills with good work habits, excellent time management and organizational skills. 
  • Familiarity with computer operation.
  • Ability to transport clients
  • Level of general health permitting stooping, bending, climbing stairs and participating in moderately active recreational activities. 
  • Physical ability to perform CPR and PMAB skills. 
  • Must be able to lift up to 30lbs.
  • Must have and maintain a level of general health which permits the performance of job duties.
