A Case Manager provides evaluation, consultation and intervention with young people and families within the community. Also participates in conducting psychosocial programming with this population as part of a clinical team.

Salary: $45,000-$52,965 Depending on experience and education
EXAMPLE OF WORK PERFORMED
- Works with young people and their families in school, mental health, home, community and/or office settings.
- Works with community and family for the purpose of creating and maintaining individualized service arrays for families.
- Provides case management and some skills training with children and families.
- Makes assessment, evaluation and recommendations for interventions and treatment planning.
- Refers families of consumers to appropriate social welfare and health agencies to obtain assistance as needed.
- Performs other related duties as required.
Experience:
- Must have a current valid Texas driver's license and current and continued proof of liability insurance.
- One year case management experience preferred.
Education:
A bachelor's degree or master’s degree in psychology, social work or related field.
KNOWLEDGE, SKILLS, AND ABILITIES
- Knowledge of emotional problems and treatment approaches.
- Ability to establish and maintain positive working relationships with consumers and colleagues.
- Good work habits and organizational skills.
- Must have excellent written/verbal communication skills.
- Familiarity with computer operation in order to do clinical documentation.
- Must have and maintain a level of general health that permits the performance of the job duties and ability to maintain regular attendance as set forth in Center policies and procedures.
- Ability to lift up to 30 lbs.
- Must be able to transport clients and travel 100% of the time.
- Level of general health permitting stooping, bending, climbing stairs and participating in moderately active recreational activities.
- Physical ability to perform CPR and PMAB skills.
