QMHP (Case Worker) will provide case management services to individuals assigned and/or court ordered (e.g. NGRI, conditions of bond, medication release) to receive outpatient mental health services with Heart of Texas Behavioral Health Network.  Caseworkers will actively monitor treatment progress of consumers and provide updates to judicial stakeholders (courts, District Attorney, etc.) as required and requested.  Caseworkers will participate in specialty court and court hearings as needed.

EXAMPLE OF WORK PERFORMED

 
  • Provide case management, skills training, and psychosocial rehabilitation services to consumers.
  • Monitors treatment progress and provides required updates on progress to judicial stakeholders (attorney's, DA's Office, and judges).
  • Performs tracking of individuals on court-ordered outpatient treatment with BHN.
  • Tracks and ensures/arranges completion of Certificate of Medical Examination(s).
  • Provides continuity of care services to consumers releasing from jail with mental health medications as well as those in the community with frequent criminal justice contact.
  • Prepares progress notes and other required documentation.
Experience:
  • One (1) year of case management experience preferred. 
  • Valid Texas driver's license, liability insurance and reliable transportation required.
Education:
Requires a bachelor’s degree or master’s degree in social work, psychology or related field with ability to obtain Qualified Mental Health Professional credentialing.

KNOWLEDGE, SKILLS, AND ABILITIES
  • Excellent written communication and basic computer knowledge for treatment planning and inputting clinical documentation. 
  • Excellent verbal communication and interpersonal relations with consumers, staff and community providers. 
  • Time management and organization are required to meet service goals. 
  • Local travel (90%) and regional travel (10%) and transport clients. 
  • Ability to learn about and implement trauma-informed care practices within the scope of work for the position. 
  • Must have knowledge to do clinical documentation.
  • Must have and maintain a level of general health that permits the performance of the job duties and ability to maintain regular attendance as set forth in Center policies and procedures.    
  • Stooping, bending, sitting, standing, lifting up to 30 lbs., and keyboarding for prolonged periods. 
  • Ability to physically perform CPR, First Aid and PMAB certification.

Salary: $45,000-$52,965 *Depending on education and experience*
