Under the immediate supervision of the Director of Integrated Health performs assigned therapy and skills training services

EXAMPLES OF WORK PERFORMED:          

 
  • Provide short-term counseling and skills training services to patients and their families in rural integrated health settings in Limestone County; provide long-term counseling as schedule and need supports.
  • Provide care coordination, system navigation, crisis intervention, and community resource linkage and referral in order to advance wellness goals for individuals served.
  • Working with an interdisciplinary team to coordinate both behavioral and physical health care in rural primary care settings.
  • Complete all documentation in an accurate and timely manner including treatment plans, treatment plan reviews, progress notes, discharge summaries, and other relevant documents.
  • Performs related work as required.
Education

Graduation from an accredited college or university with a master’s degree with a major in behavioral, social sciences, humanities, or education, as well as full licensure as a LCSW, LPC, and/or LMFT or be actively working toward licensure is required.  Clinician supervision available for those working towards licensure.

KNOWLEDGE, SKILLS AND ABILITIES:
  • Considerable knowledge of the principles, methods, techniques and practices of therapy and social work, the functioning of social service or treatment facility of community resources and how to use them effectively.
  • HOTBHN provides services in accordance with trauma-informed care guidelines, which includes cultural sensitivity and emphasizes a safe and respectful environment for all clients and staff. 
  • Experience and training in screening, assessment and treatment of trauma is preferred for all clinical positions.  
  • Must have and maintain a level of general health which permits the performance of job duties. 
  • Must be able to travel 10% of the time and have a valid driver's license and current auto insurance.
  • Ability to lift up to 15lbs.
  • Must have excellent time management and organizational skills.
  • Must have excellent verbal/written communication skills.

Hours: Mon-Fri 8am-5pm

Non-Licensed or working on licensure Salary: $48,150-$52,965 **Depending on experience
Fully Licensed Salary $64,000
