A Case manager provides evaluation, consultation and intervention with youth and families within the community.  Also participates in conducting psychosocial programming with this population as part of a clinical team.


EXAMPLES OF WORK PERFORMED
  • Works with youth and their families in school, mental health, community, home or office settings.
  • Works with community and family for the purposes of creating and maintaining individualized service arrays for families.
  • Provides case management and some skills training with children and families.
  • Make assessment and psychosocial evaluations and recommendations.
  • Refers families of consumers to appropriate social welfare and health agencies to obtain assistance.
  • Performs related work as required.

Education:
Bachelor's degree in psychology, social work or related field.

Experience
One year of case management experience Preferred.


KNOWLEDGE, SKILLS, AND ABILITIES
  • Knowledge of emotional problems and treatment approaches. 
  • Ability to establish and maintain positive working relationships with consumers and community partners and fellow staff. 
  • Good work habits including time management and organizational skills. 
  • Familiarity with computer operation. 
  • Must have excellent verbal/written communication skills.
  • Ability to transport clients 100% travel.
  • Must be able to lift up to 30lbs.
  • Must have knowledge for rehabilitation and training skills.
  • Must have assessment/diagnostic evaluation skills.
  • Must have excellent clinical documentation skills.
  •  Must have and maintain a level of general health that permits the performance of the job duties and ability to maintain regular attendance as set forth in Center policies and procedures. 
  • Level of general health permitting stooping, bending, climbing stairs and participating in moderately active recreational activities. 
  • Physical ability to perform CPR, First Aid and PMAB skills.

Salary: $21.6346-$25.4639 hourly $45,000-$52,965 Annually **Depending on education and experience**
