As the Crisis Prevention Specialist, you will have the opportunity to conduct behavioral health crisis screenings and perform follow-up services. The Crisis Prevention Specialist also works closely with the Mobile Crisis Outreach Team, Continuity of Care, as well as community stakeholders to coordinate behavioral health services to individuals following crisis hospitalization. Services include behavioral health support for up to 90 days, linkage to community supports, and care planning. The Crisis Prevention Specialist will connect individuals to other HOTBHN programs and local resources for ongoing care and recovery support.

Experience:
Requires successful experience in providing mental health services for people with severe and persistent mental illnesses, a working knowledge of psychiatric diagnosis, psychosocial assessments, and crisis intervention.
Once year of case management preferred.
Education:
Graduation from an accredited college or university with a bachelor’s degree or master’s degree, with a major in behavioral, social sciences, humanities, or education.

Salary: $21.6346-$25.4639 Hourly $45,000-$52,965 Annually **Depending on education and experience.

EXAMPLES OF WORK PERFORMED:
- Completes Follow Ups for Mobile Crisis Outreach Team and Continuity of Care following initial crisis intervention.
- Provides crisis counseling and support as deemed necessary.
- Coordinates referrals to community-based providers.
- Provides on-site crisis assessments and develops intervention plans.
- Provides crisis assessments in the community including hospitals.
- Prepares progress notes and other required documentation.
- Travels locally and regionally.

- Knowledge of mental illness and emotional components of physical illness, psychiatric diagnosis, psychosocial assessments, and crisis intervention.
- Demonstrated skill in interviewing and reporting results of interviews.
- Considerable ability to work cooperatively and effectively with professional personnel (i.e. jail staff, hospitals, shelters, etc.); to exercise tact, initiate, and good judgement in dealing with consumers and families; to prepare reports and make recommendations for administrative use; to speak and write effectively; to relate to people and to interact with consumers.
- Ability to lift up to 30lbs.
- Ability to learn about and implement trauma-informed care practices within the scope of work for the position.
- Must have excellent time management, filing skills, and clinical documentation skills.
- Ability to travel and transport clients 60% locally and 40% regionally.
- Must have and maintain a level of general health that permits the performance of the job duties and ability to maintain regular attendance as set forth in center policies and procedures.
- Ability to assist with daily living skills.
- Must have knowledge of rehabilitation/skills training and treatment planning.
