Salary - $35,000 - $39,375

Educational Requirements:  BA in Human Services with experience or equivalent experiences in a related field. 1-year post-grad experience is required.  A minimum of two years' experience is preferred. 

Certification/License Requirement:  TCM Core, SMI, SUD (Can be trained within 6 months of hire) 
Duties/Responsibilities:  The Targeted Case Manger will provide support to and advocacy for the consumers within the Dual program, as well as Outpatient Addiction services in Boyd County, as determined by the treatment team members.
Skills/Knowledge: 1-year post-grad experience required, 5 years TCM experience to bill co-occurring is preferred. Experience in Behavioral Health setting is a plus.  Case managers will display competency in working with computers and electronic devices necessary.
